Could a VCS game utilize two seperate functioning joystick/pad buttons?

There are a number of controllers that could allow this:

 

Sega Genesis

Sega Master System

Atari 7800 pro-lines or joypads

 

What do programmers think about this?

So what you're saying it IS possible to have a usable second button but it only works with certain controllers? I think might be interesting to see hacks of existing games that take advantage of that second button.
