Help MAME keyboard issue

Im trying to find a solution to something that should be very simple.  I have a controller board which is not aligned with the default MAME keyboard mapping.  I also am using this controller for FX3 Pinball as well.

 

I installed Joy2Key to map the inputs to the keyboard settings for both FX3 and MAME.

 

FX3 works perfectly however MAME doesnt work through the Windows keyboard controller and ignores any mapping - its take the raw button registers which of course dont map to the MAME defaults.

 

Im pulling my hair out trying to setup a ctrlr file that works globally.  If I change things inside a game it works just for that game, but I cant get the ctrlr file right for some reason.

 

Im using MAME224 - MAME64UI.EXE

 

Is there a simple way I can:

1.  Have MAME recognize the Windows keyboard input (JOY2KEY)

or

2.  Create a custom mapping file and have MAME use it through MAME64UI.EXE (instructions?)?
