Please identify this video game. The below image is from a recording of it.

Thank you so much. That certainly was the game for the image.

 

Unfortunately, it appears that I had wrongly associated the attached audio with that game. 

 

Any idea from which game that  bumper music came?

That song would be from Ballblazer, by Lucasfilm Games. They also did Kronos Rift, which contains a Rotofoil hulk, that when looted gives you a module that plays the tune (damaged, so slightly off key)

Yes! That is the music! Thank you so much.

 

I have no memory of that Ballblazer game. Yet, somehow, my ~40 year old memory associated its music with a game that has a similar scrolling checkerboard playing field. Odd.
