It is called Color Selector Demo (2003) (PD) in Trebor's ROM Pack. Though it is from 1984 not 2003.

 

Here is mine. I got it from Dan Kramer.

That dating got crossed with Eric Ball's Color Grid demo from 2003. 

The above will be corrected in the next update to the pack.
