Connect the wires and diodes like this (the number in the circle is the pin number on the 15-pin connector. Also, make sure the diodes are in the orientation shown): Connect the switch on your encoder to pins 4 and 6 of the 15-pin connector. Note: pin 4 is connected to 3 different places, so you will need some extra wire to connect them together. EDIT: all of the green dots below are connected together.
