Are you sure it is blank? Did you try reading the disk with an Atari drive? Or you just made an image with SCP using Splice mode?

 

If it was the latter, then it is possible that your HD drive is refusing to work without detecting an index pulse at a reasonable rate. This is similar as if you would try to read the flippy side. You will get no data at all from the drive.

 

In such case you can either make a simple ATR image with Atari hardware. Or either use an older PC (not HD) drive with SCP and Splice mode. Most older 360K PC drives don't require any index pulse at all. Of course, if you are willing to somehow hide the extra index holes as suggested, then that would be great.

Backside of Pogo Joe has 720 missing sectors, so unformatted.

 

I tried to copy Invasion Orion on real hardware using Copymate and every sector is bad or unformatted, returning.  Same with Math Busters, returning that one too.

This was expected.

That mastering error is on the disk and therefore it doesn't matter how you dump it. There will always be a second t30 sector 16 and thus a missing sector 18.

I suspect that my comment in this posting confused you (as it did ijor).
I did not question the missing sector 18 but only saw a very small chance that the duplicate sector 16 was caused by the "splice-read" - which meanwhile is proven to be wrong.

Inline with your latest dump from 4h ago, a fixed ATX which reflects the differences in track alignment between the "slice-dump" and the new "index-dump":

Downloaded it three times with no success.

7Zip tells me about header errors and creates all files but all except three with 0 bytes size.
Windows Explorer also gives an error.

Requires BASIC. I do not know how to configure Altirra to boot it.  Please reply with your configuration if you can get it to boot.

Does the physical disk work?

 

This is the copy protected version as it was released by Atari but without sector 367 being bad.

Therefore it fails protection and keeps rebooting.

This disk has physical damage on the surface of both sides.  I took several recordings.  I hope these can help with another set.

Yep, they could.

I had created a pair of Frankendisks six years ago and could happily repair your dumps taking the needed 13 sectors from them.
In the end only the two bad sectors differ between my handcrafted disks and your repaired dump. :)

Do you have a DD capable drive and did you try to boot side B there?

 

ATX currently does not support DD disks. I have not tried to convert a DD flux image but not detecting anything on a DD flux dump would be a valid option in this case.

For a diagnostic disk this might be worth the effort.

 

Oh, and I see no need for BASIC with this title.

This disk misses the needed copy protection which should be a bad sector. If you select a printer, a copyright screen shows up and the system reboots.

Weird-o-Fact:

This is the second dump of this program and the third from this company which shows this error.

 

Can you please try if this disk works on hardware?

I really don't want to believe that several non-working disks were sold.
